Job Overview: Vitium Cura Pty Ltd is seeking an experienced and compassionate Support Coordinator to assist NDIS participants in Bundaberg, QLD and surrounding areas to achieve their goals, build independence, and navigate their NDIS supports.
Key Details:
Location: Bundaberg, QLD and surrounding areas
Employment Type: Casual / Contract
Schedule: Flexible hours based on participant caseload and requirements
Your Role Will Include:
Supporting participants to understand and implement their NDIS plans
Connecting participants with suitable providers, allied health professionals, and community services
Coordinating supports in line with participants’ goals, needs, choice, and control
Monitoring plan implementation and funding utilisation
Building participants’ capacity to navigate the NDIS and their support networks
Maintaining accurate case notes, progress documentation, and stakeholder communication
Working collaboratively with participants, families, providers, and other professionals
Essential & Desired Requirements:
Previous NDIS Support Coordination experience preferred
Strong understanding of the NDIS and participant-centred practice
Relevant qualification in Social Work, Community Services, Disability, Mental Health, or a related field (desirable)
NDIS Worker Screening Clearance
Working with Children Check (WWCC)
Current First Aid & CPR Certificate
National Police Check
Driver Licence and reliable vehicle with insurance
Excellent communication, organisation, and stakeholder-management skills
Right to work in Australia
